[#20859] Document management: send service documents to any email or fax

Previously, you could send service documents by email or fax only to the referring doctor or facility (provided that their email address or fax number had been entered in the referring doctor or facility settings). With this update, we give you more flexibility by allowing you to send service documents to an arbitrary email or fax. Just type them into the corresponding fields and hit OK to send your files:

This update brings a small but useful improvement to the Billing module search filter. With the intention to help you separate studies with submitted paper insurance statements from studies with submitted paper patient statements, we have split the old statements checkbox into HCFA stmts and patient stmts respectively. You will find the new checkboxes in the Payments section of the search filter.

Also, we have added an ability to search for studies that are billed to facilities via Part A invoices and added tooltips to all of these options—hover your mouse over the checkboxes to see what each filter looks for.
